* For years I have been advising that it is a good idea to periodically check your credit report for accuracy.  You should also do the same with your medical information report.
 
The Medical Information Bureau maintains files of individual medical information, similar to a credit report, on about 25 million people.  The files are complied from information reported on applications for life, health, disability and long-term care insurance policies, and are used by approximately 600 insurance companies to make coverage decisions.
 
You can get a copy of your medical information report by sending $9.00 to:

Box 105 - Essex Station
Boston, Massachusetts 02112
 
The $9.00 fee will be waived if you have received written notification of an "adverse action" from an insurance company based on information provided by the Medical Information Bureau.
 
If you discover an error on your medical information report you can ask the Bureau to investigate the item .  Inaccurate or incomplete information will be removed or corrected.  You can also submit a "statement of dispute" to be added to the file.

* The Social Security Administration has added a new web page to its site that will connect you to everything you need to know about your Social Security number.  Go to www.ssa.gov/ssnumber to find out:
  • How to apply for a Social Security number

  • How to get a new Social Security number because of domestic abuse

If you changed your name as a result of marriage during 2002, make sure to notify the SSA of the change ASAP.

To protect your personal information and prevent identity theft, the SSA cannot issue Social Security numbers or cards online.  You still must apply in person or by mail.
